2018年世界杯足球赛中北美洲及加勒比海区资格赛第二轮定2015年9月4日到8日举行。由中北美洲及加勒比海足协排名第四档次国家队2队和10支第一轮胜队共12队进行主客场二回合交手；胜队晋级第四轮。
Seeding[编辑]
The draw for the third round was held as part of the 2018 FIFA World Cup Preliminary Draw on 25 July 2015, starting 18:00 MSK (UTC+3), at the Konstantinovsky Palace in Strelna, Saint Petersburg, Russia.[1][2]
The seeding was based on the FIFA World Rankings of August 2014 (shown in parentheses).[3] The 12 teams were seeded into two pots:
- Pot 3 contains the teams ranked 1–6.
- Pot 4 contains the teams ranked 7–12.
Each tie contained a team from Pot 1 and a team from Pot 2, with the order of legs decided by draw.

Note: Teams in italics were second round winners. Guatemala were seeded into Pot 3 as they have more FIFA ranking points (203.24) than the Saint Vincent and the Grenadines (202.53). In the FIFA World Rankings, teams share the same ranking if their ranking points round to the same whole number.
